Jon Hunter 8720fe2167 nvmap: Remove legacy configs

The NVMAP configs NVMAP_CONFIG_LOADABLE_MODULE and
NVMAP_CONFIG_UPSTREAM_KERNEL are no longer needed or used because the
NVMAP driver is now always compiled as an out-of-tree driver. Remove
these legacy configs.
