Add support for compiling nvgpu in a POSIX compliant userspace.
This code adds all of the necessary abstraction interfaces (mostly
stubbed) to enabled extremely limited and basic functionality in
nvgpu.
The goal of this code is to facilitate unit testing of the nvgpu
common core. By doing this in userspace it is much easier to write
tests that rely on very particular states within nvgpu since a user
can very precisely control the state of nvgpu.

Make the kmem debugging prints much more easily usable. Previously the
prints would only take effect if the full tracking was enabled:
CONFIG_NVGPU_TRACK_MEM_USAGE
However, there are many times when just the debug prints would be nice
to have by simply setting the log mask bit in the log mask.
echo 0x80000 > /sys/kernel/debug/<gpu>/log_mask
Also this change now uses the real nvgpu_log() function instead of the
legacy nvgpu_dbg() function. This makes the logging appear with proper
GPU printing as well.

Currently nvgpu common header files include the
Linux specific headers protected by __KERNEL__
check.
The patch includes the rmos specific headers when
the common headers (and common nvgpu files, by
extension) are compiled under QNX, so that QNX
specific data structures / type definitions can
be retrieved.

Remove the Linux includes that are included from <nvgpu/linux/kmem.h>.
These includes leak into the core code and make it easy for Linux
specific functions and APIs to be accidentally called.
The only place that includes the Linux headers is now the Linux API
implementation code.

Move the Linux specific header files to include/nvgpu/linux/ to keep
the header files more organized. Having tons of Linux specific stuff
pollute the top level include seems like a bad idea.

Implement kmem abstraction and tracking in nvgpu. The abstraction
helps move nvgpu's core code away from being Linux dependent and
allows kmem allocation tracking to be done for Linux and any other
OS supported by nvgpu.

Change nvgpu_kalloc() to nvgpu_big_[mz]alloc(). This is necessary
since the natural free function name for this is nvgpu_kfree() but
that conflicts with nvgpu_k[mz]alloc() (implemented in a subsequent
patch).
This API exists becasue not all allocation sizes can be determined
at compile time and in some cases sizes may vary across the system
page size. Thus always using kmalloc() could lead to OOM errors due
to fragmentation. But always using vmalloc() is wastful of memory
for small allocations. This API tries to alleviate those problems.

Instead of using a single static kmem_cache for each type of
data structure the allocators may want to allocate each
allocator now has its own instance of the kmem_cache. This is
done so that each GPU driver instance can accurately track how
much memory it is using.
In order to support this on older kernels a new NVGPU API has
been made,
nvgpu_kmem_cache_create(struct gk20a *g, size_t size)
To handle the possibility that caches cannot be created with
the same name.
This patch also fixes numerous places where kfree() was wrongly
used to free kmem_cache allocs.
