nvgpu_channel_get_max_subctx_count() right now always returns max subctx
count for 0th instance. Update this function to return max subctx count
for GPU instance for which channel is allocated.
For CE channels that are allocated and managed by nvgpu, cdev pointer is
not set in channel private data (since it is assigned in OS specific
code). For those channels continue returning max subctx count for 0th
instance. CE channels should not need subcontexts anyways.
Add nvgpu_cdev pointer in struct nvgpu_channel_linux. Assign it in
__gk20a_channel_open() and clear it in gk20a_channel_release()
Move code to get runlist and gpu_instance_id after nvgpu_get() call.
Accesses to gk20a pointer should always come after nvgpu_get().
Also add a debug print to dump runlist_id and gpu_instance_id being
used for channel.

Split up nvgpu_channel_clean_up_jobs() on the clean_all parameter so
that there's one version for the asynchronous ("deferred") cleanup and
another for the synchronous deterministic cleanup that occurs in the
submit path.
Forking another version like this adds some repetition, but this lets us
look at both versions clearly in order to come up with a coherent plan.
For example, it might be feasible to have the light cleanup of pooled
items in also the nondeterministic path, and deferring heavy cleanup to
another, entirely separated job queue.

Add a generic profiler based on the channel kickoff profiler. This
aims to provide a mechanism to allow engineers to (more) easily profile
arbitrary software paths within nvgpu.
Usage of this profiler is still primarily through debugfs. Next up is
a generic debugfs interface for this profiler in the Linux code.
The end goal for this is to profile the recovery code and generate
interesting statistics.

Import userd and gpfifo buffers from userspace if provided via
NVGPU_IOCTL_CHANNEL_ALLOC_GPFIFO_EX. Also supply the work submit token
(i.e., the hw channel id) to userspace.
To keep the buffers alive, store their dmabuf and attachment/sgt handles
in nvgpu_channel_linux. Our nvgpu_mem doesn't provide such data for
buffers that are mainly in kernel use. The buffers are freed via a new
API in the os_channel interface.
Fix a bug in gk20a_channel_free_usermode_buffers: also unmap the
usermode gpfifo buffer.

gk20a_submit_channel_gpfifo() supports reading the gpfifo entries from
either a kernel buffer or an userspace buffer in an ioctl. Add two
separate entry points: one for the ioctl and another for any other
kernel use. This shortens the function prototypes and simplifies and
clarifies the call sites slightly.
