NVGPU_SUBMIT_GPFIFO_FLAGS_RESCHEDULE_RUNLIST causes host to expire
current timeslice and reschedule from front of runlist.
This can be used with NVGPU_RUNLIST_INTERLEAVE_LEVEL_HIGH to make a
channel start sooner after submit rather than waiting for natural
timeslice expiration or block/finish of currently running channel.

When two or more apps ran simultaneously
First app context sets power_on flag & starts init
Other app context check the power_on flag
and try to use GPU without init completed
Which makes aother apps to assert

Currently callbacks from the PM_QOS framework (for
thermal events), result in a RPC call to set GPU frequency.
Since the governor will now be responsible for setting desired
rate, the max PM_QOS callback will now cap the possible
GPU frequency w/ a new RPC call to the server. The server
is responsible for setting the ultimate frequency
based on the cap & desired rates.

In order to perform timestamps correlation for FECS
traces, we need to collect GPU / GPU timestamps
samples. In virtualization case, it is possible for
a guest to get GPU timestamps by using read_ptimer.
However, if the CPU timestamp is read on guest side,
and the GPU timestamp is read on vm-server side,
then it introduces some latency that will create an
artificial offset for GPU timestamps (~2 us in
average). For better CPU / GPU timestamps correlation,
Added a command to collect all timestamps on vm-server
side.

it from common code and move it to Linux module

Add corresponding header files for above modules too
And compile all of above files only if CONFIG_DEBUG_FS is set
Some more details of the changes made
- Move and rename gk20a/debug_gk20a.c to common/linux/debug.c
- Move and rename gk20a/debug_gk20a.h to include/nvgpu/debug.h
- Remove gm20b/debug_gm20b.c and gm20b/debug_gm20b.h and call
gk20a_init_debug_ops() directly from gm20b_init_hal()
- Update all debug APIs to receive struct gk20a as parameter
instead of receiving struct device pointer
- Update API gk20a_dmabuf_get_state() to receive struct gk20a
pointer instead of struct device
- Include <nvgpu/debug.h> explicitly in all files where debug
operations are used
- Remove "gk20a/platform_gk20a.h" include from HAL files
which no longer need this include
- Add new API gk20a_debug_deinit() to deinitialize debugfs
and call it from gk20a_remove()
- Move API gk20a_debug_dump_all_channel_status_ramfc() to
gk20a/fifo_gk20a.c

Begin removing all of the myriad flag variables in struct gk20a and
replace that with one API that checks for flags being enabled or
disabled. The API is as follows:
bool nvgpu_is_enabled(struct gk20a *g, int flag);
bool __nvgpu_set_enabled(struct gk20a *g, int flag, bool state);
These APIs allow many of the gk20a flags to be replaced by defines.
This makes flag usage consistent and saves a small amount of memory in
struct gk20a. Also it makes struct gk20a easier to read since there's
less clutter scattered through out.

which is not portable across operating systems. The new nvgpu_err()
and nvgpu_warn() macros take struct gk20a pointer. Convert code
to use the more portable macros.

List g->pending_sema_waits already uses new nvgpu list

pending_sema_waits and a few other variables in struct gk20a is used
in gk20a shutdown path but we didn't initialize them in vgpu. I added
a function vgpu_init_vars dedicated to init variables for struct gk20a.
The native code also similar function nvgpu_init_vars().
This is a quick fix. Finally, the common probe code is better be put
in common function btween vgpu and native gpu.

This change refactors the teardown in remove to ensure that it is
possible to unload the driver while leaving fds open. This is achieved
by making sure that the SW state is kept alive till all fds are closed
and by checking that subsequent calls to ioctls after the teardown fail.
Normally, this would be achieved ny calls into gk20a_busy(), but in
kickoff we dont call into that to reduce latency, so we need to check
the driver status directly, and also in some of the functions
as we need to make sure the ioctl does not dereference the device or
platform struct

The main driver structure is not refcounted properly,
so when the driver unload, file desciptors associated to the
driver are kept open with dangling references to the main object.
This change adds referencing to the gk20a structure.

Implement kmem abstraction and tracking in nvgpu. The abstraction
helps move nvgpu's core code away from being Linux dependent and
allows kmem allocation tracking to be done for Linux and any other
OS supported by nvgpu.

Remove nvgpu_gpuid_t18x.h since this file is now visible. Migrate
the relevant definitions and defines into their expected places and
make the code use the real defines. No longer is hiding t18x specific

Implement a worker thread to replace the delayed works in channel
watchdog and job cleanups. Watchdog runs by polling the channel states
periodically, and job cleanup is performed on channels that are appended
on a work queue consumed by the worker thread. Handling both of these
two in the same thread makes it impossible for them to cause a deadlock,
as has previously happened.
The watchdog takes references to channels during checking and possibly
recovering channels. Jobs in the cleanup queue have an additional
reference taken which is released after the channel is processed. The
worker is woken up from periodic sleep when channels are added to the
queue.
Currently, the queue is only used for job cleanups, but it is extendable
for other per-channel works too. The worker can also process other
periodic actions dependent on channels.
Neither the semantics of timeout handling or of job cleanups are yet
significantly changed - this patch only serializes them into one
background thread.
Each job that needs cleanup is tracked and holds a reference to its
channel and a power reference, and timeouts can only be processed on
channels that are tracked, so the thread will always be idle if the
system is going to be suspended, so there is currently no need to
explicitly suspend or stop it.

Instead of using Linux APIs for mutex and spinlocks
directly, use new APIs defined in <nvgpu/lock.h>
Replace Linux specific mutex/spinlock declaration,
init, lock, unlock APIs with new APIs
e.g
struct mutex is replaced by struct nvgpu_mutex and
mutex_lock() is replaced by nvgpu_mutex_acquire()
And also include <nvgpu/lock.h> instead of including
<linux/mutex.h> and <linux/spinlock.h>
Add explicit nvgpu/lock.h includes to below
files to fix complilation failures.
