In the current code, gk20a.h includes io.h which gets directly included
in a lot of other files. io.h contains methods which uses a struct
gk20a as a parameter leading to a circular dependency between io.h
and gk20a.h. This can be mitigated by removing io.h from gk20a.h as
part of larger effort to moving gk20a.h to nvgpu/gk20a.h

This reverts commit 0b02c8589d.
Originally change was reverted as it was making ap_compute test on
embedded-qnx-hv e3550-t194 fail. With fixes related to replacing tsg
preempt with runlist preempt during teardown, preempt timeout set to
100 ms (earlier this was set to 1000ms for t194 and 3000ms for legacy
chips) and not issuing preempt timeout recovery if preempt fails, helped
resolve the issue.

-During polling eng preempt done, reset eng only if eng stall
intr is pending. Also stop polling for eng preempt done
if eng intr is pending.
-Add max retries for pre-si platforms for poll pbdma and eng
preempt done polling loops.

Add below two new HALs
gops.fb.enable_hub_intr() to enable hub interrupts
gops.fb.disable_hub_intr() to disable hub interrupts
Set existing APIs gv11b_fb_enable/disable_hub_intr() to these HALs
Call the HALs everywhere instead of calling the APIs directly

The following changes implements the initial (as per bringup) nvlink driver.
(1) SW initialization of nvlink core driver structures
(2) Nvlink interrupt handling
(3) Device initialization (IOCTRL, pll and clocks, device level intr)
(4) Falcon support for minion
(5) Minion load and bootstrapping
(6) Link initialization and DL PROD settings
(7) Device Interface init (and switching HSHUB to nvlink)
(8) HS set/get mode for both link and sublink
(9) Topology discovery and VBIOS settings.
(10) Ensures we get physical contiguous memory when Nvlink is enabled
This driver includes a hack for the current single dev/single link limitation.
