When building NVGPU with the GCC -Werror=sizeof-pointer-memaccess
warning enabled, the following error is seen ...

This is caused because the source buffer is passed to sizeof instead of
the destination. This could cause a buffer overflow if the source is
larger than the destination buffer.
Looking at the code further, there is another problem and that is that
after copying the string 'as_' to the 'name' buffer, the pointer 'p'
returned by strncpy is then used as the address to append an unsigned
integer to the string 'as_'. However, the pointer returned by strncpy
is actually the same address as pointed to by 'name'. Therefore, the
prefix 'as_' is actually overwritten by the call to nvgpu_strnadd_u32.
Fix these issues by initialising 'name' buffer to 'as_' statically and
then set the pointer 'p' to the offset in the 'name' buffer that
follows the prefix 'as_'. This removes the need to use strncpy at all
and simplifies the code.

- Modify NVGPU_GPU_IOCTL_ALLOC_AS and struct nvgpu_alloc_as_args to
accept start address and size of user memory. This allows configurable
address space allocation.
- Modify gk20a_as_alloc_share() and gk20a_vm_alloc_share() to receive
va_range_start and va_range_end values.
- gk20a_vm_alloc_share() initializes vm with low_hole = va_range_start,
and user vma size = (va_range_end - va_range_start).
- Modify nvgpu_as_alloc_space_args and nvgpu_as_free_space_args to
accept 64 bit number of pages.

- "include/trace/events/gk20a.h" file was having GPL2 license
(which should not used for QNX code). This file was used for
compiling linux userspace driver("libnvgpu-drv.so") and was used for
unit testing on QNX.
- This patch removes stubs in "include/trace/events/gk20a.h" file.
(which were used for linux userspace driver.)
- For QNX driver, "nvgpu_rmos/trace/events/gk20a.h" was used.
This patch moves that file to "include/nvgpu/posix/trace_gk20a.h" and
does relevant license change. This same file will be used for linux
userspace driver.
- This patch also creates a new file "include/nvgpu/trace.h" which
selects proper trace file depending on the config.

Advisory Rule 2.7 states that there should be no unused
parameters in functions.
This patch removes unused function parameters from the following:
* release_as_share_id() -> remove 'id' param
* nvgpu_pd_cache_look_up() -> remove 'g' param
* nvgpu_vm_get_pte_size_fixed_map() -> remove 'size' param

This patch fixes violations for below listed rules in
nvgpu/common/mm/as.c.
MISRA Rule 10.3 forbids implicit conversion of an essential type to
different or narrower essential type.
MISRA rule 10.4 requires essential type of left hand operand to be same
as right hand operand.
MISRA Rule 21.8 requires the size_t argument of strncpy() to be less
than length of the source string.

Make a hal/mm/gmmu sub-unit for the GMMU HAL code. Also move the
gk20a specific HAL code there. gp10b will happen in the next patch.
This change also updates all the GMMU related HAL usage, of which
there is quite a bit. Generally the only change is a .gmmu needs to
be inserted into the HAL path. Each HAL init was also updated.

The container_of() macro used in nvgpu produces the following
set of MISRA required rule violations:
* Rule 11.3 : A cast shall not be performed between a pointer to
object type and a pointer to a different object type.
* Rule 11.8 : A cast shall not remove any const or volatile
qualification from the type pointed to be a pointer.
* Rule 20.7 : Expressions resulting from the expansion of macro
parameters shall be enclosed in parentheses
Using the same modified implementation of container_of() as that
used in the nvgpu_list_node/nvgpu_rbtree_node routines eliminates
the Rule 11.8 and Rule 20.7 violations and exchanges the Rule 11.3
violation with an advisory Rule 11.4 violation.
This patch uses that same equivalent implementation in two new
(static) functions that are used to replace references to
container_of() references in vm.c:
* nvgpu_mapped_buf_from_ref
* vm_gk20a_from_ref
The implementation of the following routine has also been updated
using the same approach to eliminate the direct reference to
container_of():
* gk20a_from_as
It should be noted that replacement functions still contain
potentially dangerous (and non-MISRA compliant code) and that it is
expected that deviation requests will be filed for the new advisory
rule violations accordingly.
