Moved the following HALs
- fifo.alloc_inst
- fifo.free_inst

To channel HALs:
- channel.alloc_inst
- channel.free_inst

Moved the following fifo code:
- gk20a_fifo_alloc_inst
- gk20a_fifo_free_inst

To common channel code:
- nvgpu_channel_alloc_inst
- nvgpu_channel_free_inst

vgpu already implements
- vgpu_channel_alloc_inst
- vgpu_channel_free_inst
