MISRA rule 21.1 forbids the use of #define and #undef on a reserved
identifier or reserved macro name. Fix violations of rule 21.1 in
os_sched unit.
MISRA rule 21.2 forbids the usage of identifier names which start with
an underscore. Fix violation of MISRA rule 21.2 in os_sched unit.

Renamed nvgpu_channel_free_usermode_buffers in os/linux-channel.c to

Added NVGPU_DBG_GPU_IOCTL_SET_CTX_MMU_DEBUG_MODE ioctl to set MMU
debug mode for a given context.
Added gr.set_mmu_debug_mode HAL to change NV_PGPC_PRI_MMU_DEBUG_CTRL
for a given channel. HAL implementation for native case is
gm20b_gr_set_mmu_debug_mode. It internally uses regops, which directly
writes to the register if the context is resident, or writes to
gr context otherwise.
Added NVGPU_SUPPORT_SET_CTX_MMU_DEBUG_MODE to enable the feature.
NV_PGPC_PRI_MMU_DEBUG_CTRL has to be context switched in FECS ucode,
so the feature is only enabled on TU104 for now.

Fix 8.2 violation for not specifying parameter name in prototype of
secure_alloc().
Fix 21.3 & 21.8 violations for using reserved names "free" and "exit."
Fix 8.6 and 21.2 violations for __gk20a_do_idle() and
__gk20a_do_unidle() by renaming the functions and wrapping them in a
missing #ifdef CONFIG_PM.
Fix 5.7 violation for reusing "class" as parameter name when already
defined as a struct.

This fixes MISRA rule 4.7 violations in the function nvgpu_vm_map(). The
violations were caused by trying to use ERR_PTR() to return error
information. Rather than try to return errors in a pointer, just change
the API to return an int and pass the pointer the arguments.

MISRA Rule 21.2 prohibits naming identifiers beginning with a double
underscore. This updates the function names __nvgpu_check_gpu_state()
and __gk20a_warn_on_no_regs() to comply.

Modified PMU subunits init sequence as they required during PMU RTOS
initialize stage, function nvgpu_pmu_early_init() allocates space for
all its subunit & its default values for further usage during runtime,
and function nvgpu_pmu_init() performs setup for sub units & starts
PMU-RTOS boot.
Deinit of all sub units allocated space will handled as part of
remove_support stage & also, during init stage upon failure of
init/setup sequence.

MISRA Rule 11.2 doesn't allow conversion to or from an incomplete type
pointer, as it may result incorrect point alignment and may further lead
to undefined behavior.
MISRA Rule 16.x requires all switch statements to be well-formed with
terminating break statement for every switch-clause.
This patch fixes 11.2 and 16.x violations in common.mm.nvgpu_mem.

APIs to retrieve fbp data.

Remove gr_priv.h from outside gr files.
Add hal function in gr.init for get_no_of_sm. This helps
to avoid dereferencing gr in couple of files for g->gr->config and
avoid gr_priv.h include in those files.
Replace nvgpu_gr_config_get_no_of_sm call with
g->ops.gr.init.get_no_of_sm for files outside gr unit.

There are many miscellaneous HALs for various MM related functionality.
This patch aims to migrate all the remaining MM code from the <chip>/
mm_<chip>.[ch] files in HAL files under hal/.
Much of this is fairly straightforward copy/paste and updates to the
HAL init files.
The exception to that is the move of the left over gv11b MMU fault
handling code in mm_gv11b.c. Having both a hal/mm/mm/mm_gv11b.c and
a gv11b/mm_gv11b.c file causes tmake to choke so the gv11b/mm_gv11b.c
file was moved to gv11b/mmu_fault_gv11b.c. This will be cleaned up in
a subsequent patch.

version. To know the version of the nvgpu driver we introduce a new
global characteristic flag NVGPU_DRIVER_REDUCED_PROFILE.

The LSB is position 1 for ffs and LSB is position 0 for __fls. Fix the
