MISRA Rule 21.2 prohibits naming identifiers beginning with a double
underscore. This updates the function names __nvgpu_check_gpu_state()
and __gk20a_warn_on_no_regs() to comply.

Modified PMU subunits init sequence as they required during PMU RTOS
initialize stage, function nvgpu_pmu_early_init() allocates space for
all its subunit & its default values for further usage during runtime,
and function nvgpu_pmu_init() performs setup for sub units & starts
PMU-RTOS boot.
Deinit of all sub units allocated space will handled as part of
remove_support stage & also, during init stage upon failure of
init/setup sequence.

MISRA Rule 11.2 doesn't allow conversion to or from an incomplete type
pointer, as it may result incorrect point alignment and may further lead
to undefined behavior.
MISRA Rule 16.x requires all switch statements to be well-formed with
terminating break statement for every switch-clause.
This patch fixes 11.2 and 16.x violations in common.mm.nvgpu_mem.

Remove gr_priv.h from outside gr files.
Add hal function in gr.init for get_no_of_sm. This helps
to avoid dereferencing gr in couple of files for g->gr->config and
avoid gr_priv.h include in those files.
Replace nvgpu_gr_config_get_no_of_sm call with
g->ops.gr.init.get_no_of_sm for files outside gr unit.

There are many miscellaneous HALs for various MM related functionality.
This patch aims to migrate all the remaining MM code from the <chip>/
mm_<chip>.[ch] files in HAL files under hal/.
Much of this is fairly straightforward copy/paste and updates to the
HAL init files.
The exception to that is the move of the left over gv11b MMU fault
handling code in mm_gv11b.c. Having both a hal/mm/mm/mm_gv11b.c and
a gv11b/mm_gv11b.c file causes tmake to choke so the gv11b/mm_gv11b.c
file was moved to gv11b/mmu_fault_gv11b.c. This will be cleaned up in
a subsequent patch.

Moved the following HALs from fifo to tsg
- set_timeslice
- default_timeslice_us
Renamed

Scale timeslice to take into account PTIMER clock in
nvgpu_runlist_append_tsg.
Removed gk20a_channel_get_timescale_from_timeslice, and
instead moved timeout and scale computation into runlist HAL,
when building TSG entry:
- runlist.get_tsg_entry
Use ram_rl_entry_* accessors instead of hard coded values
for default and max timeslices.
Added #defines for min, max and default timeslices.

max_ctxsw_ring_buffer_size variable in struct nvgpu_gr is used to store
max ring buffer size which is then referred into linux specific code
We only use macro GK20A_CTXSW_TRACE_MAX_VM_RING_SIZE to initialize
the variable.
And max_ctxsw_ring_buffer_size does not belong to nvgpu_gr struct
anyways
Considering above remove max_ctxsw_ring_buffer_size from nvgpu_gr and
use macro directly in linux specific code

When flashing OBE ROM on PG189 with an internal chip SKU, it
ends up enumerating with unsupported device id. Since VBIOS
update will be done automatically in PDK, we want to detect
such boards, and throw a warning.
Use upper bits of driver_data to define some flags for pci device.
Added PCI_DEVICE_F_INTERNAL_CHIP_SKU for boards with internal
chip SKUs.
Allow enumeration of PG189 boards with internal chip SKUs
0x1eae and 0x1eaf, but throw a warning.

cyclestats_snapshot data and lock is right now stored in struct nvgpu_gr
Use case itself is not specific to GR engine but in general it applies
to other units outside of GR too.
Hence it makes sense to move both data and lock to struct gk20a instead
of keeping them in struct nvgpu_gr
Update all cyclestats_snapshot code to refer data/lock from struct gk20a
Remove gr_priv.h header include from cyclestats_snapshot.c
Some of the functions were mistakenly declared in gr_gk20a.h.
Move them to cyclestats_snapshot.h and rename them to form nvgpu_css_*()

gr_gk20a.ctx_vars struct right now stores sizes for golden_image, zcull,
pm_ctxsw, and gfxp_preemption_buffer.
but these sizes should be really owned by respective units and should
be assigned to units as soon as they are queried from FECS
Add new structure to nvgpu_gr_falcon to hold sizes that will be queried
from FECS

and then above APIs will be used to set sizes into respective units
All of above means size variables in gr_gk20a.ctx_vars struct are no more
being referred. Delete them.

struct gr_gk20a defines boolean flag golden_image_initialized to
indicate if golden_image is initialized or not
common.gr.obj_ctx also added a flag of its own to check if golden_image
is ready
Add new API nvgpu_gr_obj_ctx_is_golden_image_ready() in
common.gr.obj_ctx unit to get status of golden_image
Use this new API everywhere to check if golden image is ready
Remove g->gr.ctx_vars.golden_image_initialized
Also remove ctx_mutex from struct gr_gk20a
Add new flag golden_image_initialized to struct nvgpu_pmu_pg and set it
when golden image is initialized. This is needed to avoid circular
dependency between GR and PMU

This change also updates all the GMMU related HAL usage, of which
there is quite a bit. Generally the only change is a .gmmu needs to
be inserted into the HAL path. Each HAL init was also updated.
