54b11a456c gpu: nvgpu: Fix MISRA 21.2 violations [1/3]
MISRA 21.2 states that we may not use reserved identifiers; since
all identifiers beginning with '_' are reserved by libc, the usage
of '__' as a prefix is disallowed.

This change removes the usage of the '__a' argument scattered
throughout the nvgpu allocator code.

05f45bcfc3 gpu: nvgpu: MISRA 10.3 Conversions to/from char
MISRA Rule 10.3 states that the value of an expression shall not be
assigned to an object with a narrower essential type or of a
different essential type category.

We have cases where we are converting to/from char and non char types
and this fix 10.3 violations resulting from these conversions.

This also fix violations in conversions between s8 and non-s8 types
as s8 can be typedefed as char.

553fdf3534 gpu: nvgpu: common: mm: Fix MISRA 15.6 violations
MISRA Rule-15.6 requires that all if-else blocks be enclosed in braces,
including single statement blocks. Fix errors due to single statement
if blocks without braces, introducing the braces.

98e87ca10c gpu: nvgpu: POSIX does not have a strlcpy
So don't use it in common code. This could be implemented in
common code but it would just be a wrapper around strncpy()
most likely since we aren't going to maintain low level
(possibly asm) implementations of APIs.

8f2f979428 gpu: nvgpu: cleanup allocator debugging
Remove debugging features that did not really get used and make
the debugging code use the nvgpu_log() functionality. This ties
the allocator debugging into the larger nvgpu debug framework.

Also modify many of the places CONFIG_DEBUG_FS was used to
conditionally compile allocator debug code to use __KERNEL__
instead. This is because that debug code can still be called even
when debugfs is not present in Linux.

6090a8a7ee gpu: nvgpu: move debugfs code to linux module
Since all debugfs code is Linux specific, remove
it from common code and move it to Linux module

Debugfs code is now divided into below
module specific files :

common/linux/debug.c
common/linux/debug_cde.c
common/linux/debug_ce.c
common/linux/debug_fifo.c
common/linux/debug_gr.c
common/linux/debug_mm.c
common/linux/debug_allocator.c
common/linux/debug_kmem.c
common/linux/debug_pmu.c
common/linux/debug_sched.c

Add corresponding header files for above modules too
And compile all of above files only if CONFIG_DEBUG_FS is set

Some more details of the changes made

- Move and rename gk20a/debug_gk20a.c to common/linux/debug.c
- Move and rename gk20a/debug_gk20a.h to include/nvgpu/debug.h

- Remove gm20b/debug_gm20b.c and gm20b/debug_gm20b.h and call
  gk20a_init_debug_ops() directly from gm20b_init_hal()

- Update all debug APIs to receive struct gk20a as parameter
  instead of receiving struct device pointer
- Update API gk20a_dmabuf_get_state() to receive struct gk20a
  pointer instead of struct device

- Include <nvgpu/debug.h> explicitly in all files where debug
  operations are used
- Remove "gk20a/platform_gk20a.h" include from HAL files
  which no longer need this include

- Add new API gk20a_debug_deinit() to deinitialize debugfs
  and call it from gk20a_remove()
- Move API gk20a_debug_dump_all_channel_status_ramfc() to
  gk20a/fifo_gk20a.c

9db45cf037 gpu: nvgpu: Fix build failure in allocator.h
Fix a build failure caused by missing definition for struct device.
Instead of including device.h wrap the debugfs init function with
CONFIG_DEBUG_FS and forward declare struct device. We don't use any
struct device internals here so we only need to let the compiler
know that this type does exist.

b7fca01b0e gpu: nvgpu: Scrub gk20a_platform dependencies
Remove gk20a_platform dependencies from gk20a.h. This makes
gk20a_platform a Linux platform specific data structure. Add #include
for platform_gk20a.h in the source files that still depend on Linux.

b3e1ce04b9 gpu: nvgpu: Put debugfs dependencies inside #ifdef
Put all debugfs dependencies inside #ifdef CONFIG_DEBUG_FS. This
includes some functions in allocators that were used only for
debugging.

Remove include of linux/debugfs.h on files that do not deal with
debugfs.

linux/debugfs.h implicitly included linux/fs.h, which we relied on.
Add explicit include of linux/fs.h for all files where this is the
case.

8ee3aa4b31 gpu: nvgpu: use common nvgpu mutex/spinlock APIs
Instead of using Linux APIs for mutex and spinlocks
directly, use new APIs defined in <nvgpu/lock.h>

Replace Linux specific mutex/spinlock declaration,
init, lock, unlock APIs with new APIs
e.g
struct mutex is replaced by struct nvgpu_mutex and
mutex_lock() is replaced by nvgpu_mutex_acquire()

And also include <nvgpu/lock.h> instead of including
<linux/mutex.h> and <linux/spinlock.h>

Add explicit nvgpu/lock.h includes to below
files to fix complilation failures.

d630f1d99f gpu: nvgpu: Unify the small and large page address spaces
The basic structure of this patch is to make the small page allocator
and the large page allocator into pointers (where they used to be just
structs). Then assign each of those pointers to the same actual
allocator since the buddy allocator has supported mixed page sizes
since its inception.

For the rest of the driver some changes had to be made in order to
actually support mixed pages in a single address space.

1. Unifying the allocation page size determination

   Since the allocation and map operations happen at distinct
   times both mapping and allocation of GVA space must agree
   on page size. This is because the allocation has to separate
   allocations into separate PDEs to avoid the necessity of
   supporting mixed PDEs.

   To this end a function __get_pte_size() was introduced which
   is used both by the balloc code and the core GPU MM code. It
   determines page size based only on the length of the mapping/
   allocation.

2. Fixed address allocation + page size

   Similar to regular mappings/GVA allocations fixed address
   mapping page size determination had to be modified. In the
   past the address of the mapping determined page size since
   the address space split was by address (low addresses were
   small pages, high addresses large pages). Since that is no
   longer the case the page size field in the reserve memory
   ioctl is now honored by the mapping code. When, for instance,
   CUDA makes a memory reservation it specifies small or large
   pages. When CUDA requests mappings to be made within that
   address range the page size is then looked up in the reserved
   memory struct.

   Fixed address reservations were also modified to now always
   allocate at a PDE granularity (64M or 128M depending on
   large page size. This prevents non-fixed allocations from
   ending up in the same PDE and causing kernel panics or GMMU
   faults.

3. The rest...

   The rest of the changes are just by products of the above.
   Lots of places required minor updates to use a pointer to
   the GVA allocator struct instead of the struct itself.

Lastly, this change is not truly complete. More work remains to be
done in order to fully remove the notion that there was such a thing
as separate address spaces for different page sizes. Basically after
this patch what remains is cleanup and proper documentation.

6df3992b60 gpu: nvgpu: Move allocators to common/mm/
Move the GPU allocators to common/mm/ since the allocators are common
code across all GPUs. Also rename the allocator code to move away from
gk20a_ prefixed structs and functions.

This caused one issue with the nvgpu_alloc() and nvgpu_free() functions.
There was a function for allocating either with kmalloc() or vmalloc()
depending on the size of the allocation. Those have now been renamed to
nvgpu_kalloc() and nvgpu_kfree().
