nvgpu_has_syncpoints is more general than a channel synchronization
related, so move it to nvhost.c from channel_sync.c. Move the
declaration from gk20a.h to nvhost.h.
As the debugfs knob is Linux related, move it from struct gk20a to
struct nvgpu_os_linux.

Whitelist 2 MISRA Rule 17.2 violations in MM that were approved as
deviations in TID-278. These two violations are for recursive functions
that handle page table descriptors in the GMMU page table. Both cases
are tightly controlled recursion by limiting the recursion depth to the
number of possible page table levels in the hardware. For current
hardware that is a maximum recursion depth of 5 which is easily an
acceptable depth and should cause no stack issues.

The two MM functions nvgpu_set_pd_level() and nvgpu_vm_do_free_entries()
are simple recursive functions for processing page descriptors (PDs) by
traversing the levels in the PDs. MISRA Rule 17.2 prohibits functions
calling themselves because "unless recursion is tightly controlled, it
is not possible to determine before execution what the worst-case stack
usage could be." So, this change limits the recursion depth of each of
these functions to the maximum number of page table levels by checking
the level against the MM HAL max_page_table_levels().
This also required configuring this HAL in various unit tests as they
were no previously using this HAL.

When reviewing some VM mapping code I found the name of the function
that actually maps the passed buffer to be confusing. It was:
nvgpu_vm_map_compression_comptags()
This function does actually do the map and work out some comptags
related stuff. But I think this would be easier to read in the code
as:
nvgpu_vm_do_map()
As this is much more literally what the function is actually doing.

There are 2 instances where some code is unreachable in vm.c
- The nvgpu_insert_mapped_buf function always returned 0, so any
associated error handling was unreachable. This patch changes
the function to return void instead.
- A cleanup section to unmap a buffer in case of error was also
unreachable.

Add macros for whitelisting coverity violations. These macros use pragma
directives. The pragma directives and whitelisting macros are only
enabled when a coverity scan is being run.
The whitelisting macros have been added to a new header called
static_analysis.h. The contents of safe_ops.h (CERT C safe ops) have
been moved into static_analysis.h because this will be the new header
for static analysis related macros/defines/etc.

For getting the buffer size qnx issues a devctl to nvmap which can fail
as well. So, check the size that is returned by nvgpu_os_buf_get_size.
If 0 size is returned then return -EINVAL to the caller.

presence of unused variable assignments may indicate error in program's
logic.
Rule 21.x doesn't allow reserved identifier or macro names starting with

Move the PTE size computation from MM to VM where it belongs. This
function is only ever used in vm.c so move it there and make it
static.
This is part of the broader effort to cleanup the top level unit
header files and only expose functions needed by other units in the
top level unit header files.

Make the nvgpu_init_mutex function return void.
In linux case, this doesn't affect anything since mutex_init
returns void.
For posix, we assert() and die if pthread_mutex_init fails.
This alleviates the need to error inject for _every_
nvgpu_mutex_init function in the driver.

The vm object num_user_mapped_buffers was declared as an int. However,
it is an unsigned value. Being a signed value required a cast to
unsigned when calling nvgpu_big_zalloc() which causes a CERT-C INT31
violation. So, avoid the cast and use an unsigned type. And fix related
INT30 violations related to num_user_mapped_buffers as well.
To avoid introducing new MISRA/CERT-C violations, update the upstream
user of these changes, fifo/channel.c and make the equivalent uses of
this value, num_mapped_buffers a u32 as well.

Rule INT31 requires integer conversions do not result in losing or
misinterpreting data. For most cases, use the safe cast operations.
For one case, the conditional operator was being used for s16 values
which were being promoted to ints. So, replace the conditional operator
with an if statement.

Previously initializing buddy_allocator with size=0 initialized large
memory block. With fixes in buddy_allocator_init() function, size = 0
triggered segmentation fault and so size was temporarily updated to
fixed value.
This patch updates buddy_allocator_init() function to return error if
requested size of buddy_allocator is zero. As kernel VMA is absent for
VGPU, this patch also updates nvgpu_vm_do_init() function to not
allocate kernel VMA with size = 0.

This fixes MISRA rule 4.7 violations in the function nvgpu_vm_map(). The
violations were caused by trying to use ERR_PTR() to return error
information. Rather than try to return errors in a pointer, just change
the API to return an int and pass the pointer the arguments.

Fix Rule 4.7 violation for failing to check return value.
Fix Rule 14.2 violation when using the nvgpu_list_for_each_entry_safe
macro by refactoring to use a while loop.
Fix Rule 13.5 violation for using a nvgpu_timeout_expired_msg() in a
logical expression because it can have side effects. This was fixed by
refactoring the while loop.

MISRA rule 2.2 defines dead code as "operations which are executed but
removal of these operations has no effect on program behavior".
Variable initializations violate this rule if initialized value is not
used and replaced.
This patch fixes some of these reported violations.

There are many miscellaneous HALs for various MM related functionality.
This patch aims to migrate all the remaining MM code from the <chip>/
mm_<chip>.[ch] files in HAL files under hal/.
Much of this is fairly straightforward copy/paste and updates to the
HAL init files.
The exception to that is the move of the left over gv11b MMU fault
handling code in mm_gv11b.c. Having both a hal/mm/mm/mm_gv11b.c and
a gv11b/mm_gv11b.c file causes tmake to choke so the gv11b/mm_gv11b.c
file was moved to gv11b/mmu_fault_gv11b.c. This will be cleaned up in
a subsequent patch.

This "HAL" exists to handle the vGPU specific bind channel operation.
This patch moves the native function implementation to common/mm/vm.c
and renames the gk20a to nvgpu to follow the convention for vGPU vs

Currently, buddy, page and bitmap allocators have individual init()
functions. This patch creates common nvgpu_alloc_allocator_init()
function to trigger the individual functions based on allocator type
argument. This makes writing requirements for the allocators easier.

Make a hal/mm/gmmu sub-unit for the GMMU HAL code. Also move the
gk20a specific HAL code there. gp10b will happen in the next patch.
This change also updates all the GMMU related HAL usage, of which
there is quite a bit. Generally the only change is a .gmmu needs to
be inserted into the HAL path. Each HAL init was also updated.

Moved cbc related code and data from gr to cbc unit.
Ltc and cbc related data is moved from gr header:
1. Ltc related data moved from gr_gk20a -> gk20a and it
will be moved eventually to ltc unit:

Modified code to use above updated data structures.
Removed cbc init sequence from gr and added in
common cbc unit. This sequence is getting called
from common nvgpu init code.
