Add a gops_gin struct for handling chip-specific GIN functionality. For
now, it only has the get_intr_ctrl_msg HAL. Each per-unit INTR_CTRL
register holds an opaque 32-bit value that determines how an interrupt
is routed. The layout of this value is determined by GIN. This HAL
allows units to ask GIN how it should program their INTR_CTRL registers.

nvgpu_gr_ctx_mappings 1<---->n nvgpu_gr_subctx (ALLOC_OBJ_CTX)
nvgpu_gr_ctx_mappings 1<---->1 vm_gk20a (ALLOC_OBJ_CTX)
On unbinding the channel, objects are deleted according
to dependencies.
Without subcontexts, gr_ctx buffers mappings are maintained in the
struct nvgpu_gr_ctx. For subcontexts, they are maintained in the
struct nvgpu_gr_subctx.
Preemption buffer with index NVGPU_GR_CTX_PREEMPT_CTXSW and PM
buffer with index NVGPU_GR_CTX_PM_CTX are to be mapped in all
subcontexts when they are programmed from respective ioctls.
Global GR context buffers are to be programmed only for VEID0.
Based on the channel object class the state is patched in
the patch buffer in every ALLOC_OBJ_CTX call unlike
setting it for only first channel like before.
PM and preemptions buffers programming is protected under TSG
ctx_init_lock.
tsg->vm is now removed. VM reference for gr_ctx buffers mappings
is managed through gr_ctx or gr_subctx mappings object.
For vGPU, gr_subctx and mappings objects are created to reference
VMs for the gr_ctx lifetime.
The functions nvgpu_tsg_subctx_alloc_gr_subctx and nvgpu_tsg_-
subctx_setup_subctx_header sets up the subcontext struct header
for native driver.
The function nvgpu_tsg_subctx_alloc_gr_subctx is called from
vgpu to manage the gr ctx mapping references.
free_subctx is now done when unbinding channel considering
references to the subcontext by other channels. It will unmap
the buffers in native driver case. It will just release the
VM reference in vgpu case.
Note that TEGRA_VGPU_CMD_FREE_CTX_HEADER ioctl is not called
by vgpu any longer as it would be taken care by native driver.

In order to maintain separate mappings of GR TSG and global context
buffers for different subcontexts, we need to separate the memory
struct and the mapping struct for the buffers. This patch moves
the mappings of all GR ctx buffers to new structure
nvgpu_gr_ctx_mappings.
This will be instantiated per subcontext in the upcoming patches.
Summary of changes:
1. Various context buffers were allocated and mapped separately.
All TSG context buffers are now stored in gr_ctx->mem[] array
since allocation and mapping is unified for them.
2. Mapping/unmapping and querying the GPU VA of the context
buffers is now handled in ctx_mappings unit. Structure
nvgpu_gr_ctx_mappings in nvgpu_gr_ctx holds the maps.
On ALLOC_OBJ_CTX this struct is instantiated and deleted
on free_gr_ctx.
3. Introduce mapping flags for TSG and global context buffers.
This is to map different buffers with different caching
attribute. Map all buffers as cacheable except
PRIV_ACCESS_MAP, RTV_CIRCULAR_BUFFER, FECS_TRACE, GR CTX
and PATCH ctx buffers. Map all buffers as privileged.
4. Wherever VM or GPU VA is passed in the obj_ctx allocation
functions, they are now replaced by nvgpu_gr_ctx_mappings.
5. free_gr_ctx API need not accept the VM as mappings struct
will hold the VM. mappings struct will be kept in gr_ctx.
6. Move preemption buffers allocation logic out of
nvgpu_gr_obj_ctx_set_graphics_preemption_mode.
7. set_preemption_mode and gr_gk20a_update_hwpm_ctxsw_mode
functions need update to ensure buffers are allocated
and mapped.
8. Keep the unit tests and documentation updated.
With these changes there is clear seggregation of allocation and
mapping of GR context buffers. This will simplify further change
to add multiple address spaces support. With multiple address
spaces in a TSG, subcontexts created after first subcontext
just need to map the buffers.

- Replaces nvgpu_report_*_err with nvgpu_report_err_to_sdl.
- Removes the reporting of error messages.
- Replaces nvgpu_log() with nvgpu_err(), for error reporting.
- Removes error reporting to Safety_Services from nvgpu_report_*_err.

Add a new Central Interrupt Controller(CIC) unit in common code.
The interrupt handling is done in a distributed manner currently.
The error handling policy for different errors resides in each unit's
ISR code. The goal is to converge this data under one central place -
the CIC unit.
This patch creates framework for CIC unit and moves the gv11b QNX
safety LUT to CIC unit. All the error reporting APIs from different
units are also moved to CIC.
New APIs are exposed by CIC unit to access its internal data like:
1. Struct err_desc - the static err handling /injection data per
error id
2. Num_hw_modules - the number of error reporting HW units
supported by CIC
Init and deinit of CIC unit:
1. CIC unit should be initialized earlyon during boot so that it
is available for any interrupt handling.
2. Initialize CIC just before the interrupts are enabled during
boot.
3. Similarly, CIC is disabled late during deinit cycle; right
after the interrupts are masked.
LUT:
1. LUT is currently used only for reporting error to safety
services in gv11b QNX safety build.
2. This error handling policy LUT currently has only two levels
of handing - correctable and quiecse.
3. Once, the error handling policy decision is moved from leaf
unit nodes to CIC, LUT will be updated to have additional levels
like fast recovery and full recovery.
4. Also, then a separate LUT will be added for each platform/build.
5. In current framework, the LUT is set to NULL for all
configurations except gv11b.
report_err() ops is added to report error to safety services.
This ops is only effective for gv11b qnx build; and set to NULL for
other configurations.

The SDL's error reporting code will be leveraged by central interrupt
controller (CIC) or common.cic unit.
This is a base patch to move SDL error reporting code from QNX
to common. Move the data structures used during error reporting to
common header - nvgpu_err_info.h

- mc.reset_engine()
- mc.reset_engine_enable()
Add new mc hals
- mc.enable_units(g, units, enable)
> enable/disable given unit(s)
- mc.enable_dev(g, dev, enable)
> enable/disable engine represented by given device pointer
- mc.enable_devtype(g, devtype)
> enable/disable all engines of given devtype
Move common mc intr functions to common/mc/mc_intr.c.
Add below common mc functions
- nvgpu_mc_reset_units(g, units)
> reset given logical OR of nvgpu unit bitmap
- nvgpu_mc_reset_dev(g, dev)
> reset given single engine via dev
> if engine is graphics, reset gpcs for nvgpu_next
- nvgpu_mc_reset_devtype(g, devtype)
> reset all engines of given devtype
> if devtype is graphics, reset gpcs for nvgpu_next

Add a new header file <nvgpu/gr/gr_instances.h> that supports below
macros to execute various functions for GR instances
1) nvgpu_gr_exec_for_each_instance
Execute a function for each GR instance by configuring GR remap
window for that instance. Function being executed returns void.
2) nvgpu_gr_exec_with_ret_for_each_instance
Execute a function for each GR instance by configuring GR remap
window for that instance. Function being executed returns an error.
3) nvgpu_gr_exec_for_all_instances
Execute a function for all GR instances at once. For this GR remap
window needs to be disabled temporarily.
If CONFIG_NVGPU_MIG is disabled, all above macros will turn into simple
funciton calls.
If CONFIG_NVGPU_MIG is disabled or if runtime flag NVGPU_SUPPORT_MIG is
disabled, all above macros will turn into simple function calls that
configure single GR instance.
Separate out GR engine reset code into new API gr_reset_engine() and
execute it with nvgpu_gr_exec_with_ret_for_each_instance().
PROD values need to be loaded in legacy mode, hence call
nvgpu_cg_init_gr_load_gating_prod() inside
nvgpu_gr_exec_for_all_instances().
Rename gr_init_prepare_hw() to more appropriate
gr_reset_hw_and_load_prod()
Moe gops.gr.init.fifo_access() call to gr_init_reset_enable_hw().
Add new API nvgpu_grmgr_get_gr_syspipe_id() to query GR instance syspipe
id from common.grmgr unit. Add nvgpu_gr_get_syspipe_id() that returns
same value stored in nvgpu_gr struct.
Add cur_gr_instance field to struct nvgpu_gr to track current GR
instance being programmed under remap window.

Following changes are made in this patch.
1) Change unnamed structs within gpu_ops to named structs
with the prefix gops_*.
2) Each named struct gops_ are moved into a separate gops specific file
under include/nvgpu/gops/
3) struct gpu_ops is moved into a separate file include/nvgpu/gpu_ops.h
and all other dependent struct gops_* are included in this header.
4) Direct references to include/nvgpu/gops are removed from files as its enough
to include gk20a.h.

* Removed unnecessary irqs_enabled flag, and
Replaced enable/disable irq logics with nvgpu variant functions.
* Added nvgpu_interrupts data structure to hold interrupt details.
* Interpret all stall irqs first and followed by nonstall irq from dt.
* Used interrupt size checks for enable/disable irqs instead of
comparing stall and nonstall interrupt lines.
Now adding new stall interrupt lines as easy as just updating macro.

Continue making the incoherent channel functionality more structured by
moving the worker thread business to one file and the channel watchdog
logic to another. This is channel-internal restructuring; the interface
to other units does not change.
The watchdog logic is called from the worker thread and as such these
are rather tightly coupled but it's possible to have the thread and not
the watchdog.

The lockless allocator that spins in alloc and free ops using cmpxchg to
mitigate race conditions has only ever been used for the post fences in
preallocated job resources. Now each post fence has a clear owner (the
job struct which already is allocated well) and lifetime, so this
allocator has no longer a purpose. Delete it to avoid bitrot. (The
design of the job queue has always been such that there's minimal
contention in any case.)
