A comment for gk20a_fifo_update_runlist() says:
/* add/remove a channel from runlist
special cases below: runlist->active_channels will NOT be changed.
(ch == NULL && !add) means remove all active channels from runlist.
(ch == NULL && add) means restore all active channels on runlist. */
Those special cases call for a new function, so add that. Delete the
update_runlist HAL op and add update_for_channel (like update_runlist
without the special cases) and reload (no channel to add or remove, just
the special cases).
While at it, rename gk20a_fifo_update_runlist_ids to
nvgpu_runlist_reload_ids. It's common across chips and does what the
reload HAL does but for a list of several IDs.

gk20a_fifo_update_runlist_locked() and the vgpu counterpart do three
things:
1. find out whether there's a channel to add or remove (if not, the
whole runlist is just reconstructed or cleared),
2. reconstruct the runlist format for hardware (or the vgpu message),
3. actually update the runlist to hw and maybe wait for finish.
Split out the two first operations to separate functions to make the
code easier to understand. Now it's also clearer that the "add"
parameter behaves completely differently depending on whether the
channel pointer is NULL or not.
Also ignore (with a warning) channels not bound to a tsg. We shouldn't
get runlist updates on such channels. This simplifies the control flow a
bit.

A naked channel ID does not carry good information about the channel
validity and is a very low level construct for an API of this level.
Refactor the runlist updating fifo APIs to take a channel pointer.
While at it, delete the channel and wait_for_finish parameters from
gk20a_fifo_update_runlist_ids() - the only caller is suspend and resume
and the parameters were always null for channel and true for wait.

We had to force allocation of physically contiguous memory for
USERD in nvlink case, as a channel's USERD address is computed as
an offset from fifo->userd address, and nvlink bypasses SMMU.
With 4096 channels, it can become difficult to allocate 2MB of
physically contiguous sysmem for USERD on a busy system.
PBDMA does not require any sort of packing or contiguous USERD
allocation, as each channel has a direct pointer to that channel's
512B USERD region. When BAR1 is supported we only need the GPU VAs
to be contiguous, to setup the BAR1 inst block.
- Add slab allocator for USERD.
- Slabs are allocated in SYSMEM, using PAGE_SIZE for slab size.
- Contiguous channels share the same page (16 channels per slab).
- ch->userd_mem points to related nvgpu_mem descriptor
- ch->userd_offset is the offset from the beginning of the slab
- Pre-allocate GPU VAs for the whole BAR1
- Add g->ops.mm.bar1_map() method
- gk20a_mm_bar1_map() uses fixed mapping in BAR1 region
- vgpu_mm_bar1_map() passes the offset in TEGRA_VGPU_CMD_MAP_BAR1
- TEGRA_VGPU_CMD_MAP_BAR1 is called for each slab.

preempt_channel needs to use the channel to pass it to other
public functions, get access to a tsg etc. This qualifies it to take a
pointer to a channel as an input parameter instead of a chid.
Increment the channel ref counter using the function
gk20a_channel_from_id in functions where we get the chid from the h/w
registers directly. Once the prempt_channel function call is done,
use a gk20a_channel_put on the referenced channel.

gv11b_fifo_preempt_tsg needs to access the runlist_id of the tsg as
well as pass the tsg pointer to other public functions such as
gk20a_fifo_disable_tsg_sched. This qualifies the preempt_tsg to use a
pointer to a struct tsg_gk20a instead of just using the tsgid.

Add gk20a_channel_from_id() to retrieve a channel, given a raw channel
ID, with a reference taken (or NULL if the channel was dead). This makes
it harder to mistakenly use a channel that's dead and thus uncovers bugs
sooner. Convert code to use the new lookup when applicable; work remains
to convert complex uses where a ref should have been taken but hasn't.
The channel ID is also validated against FIFO_INVAL_CHANNEL_ID; NULL is
returned for such IDs. This is often useful and does not hurt when
unnecessary.
However, this does not prevent the case where a channel would be closed
and reopened again when someone would hold a stale channel number. In
all such conditions the caller should hold a reference already.
The only conditions where a channel can be safely looked up by an id and
used without taking a ref are when initializing or deinitializing the
list of channels.

Currently has_timedout variable is protected by wmb at places
where it is being set and there is no correspoding rmb whenever
has_timedout variable is read. This is prone to errors for
concurrent execution. This change is supposed to fix this issue.
Rename has_timedout variable of channel struct to ch_timedout.
Also to avoid rmb every time ch_timedout is read,
ch_timedout_spinlock is added to protect ch_timedout
variable for taking care of concurrent execution.

MISRA Rule 21.15 prohibits use of memcpy() with incompatible ptrs
to qualified/unqualified types.
To circumvent this issue we've introduced a new MISRA-compliant
nvgpu_memcpy() function.
This change switches non-offending memcpy usage in vgpu/* code
over to to use nvgpu_memcpy() with appropriate casts applied
to maintain consistency within nvgpu.

MISRA Rule-17.7 requires the return value of all functions to be used.
Fix is either to use the return value or change the function to return
void. This patch contains fix for all 17.7 violations instandard C functions
in GPU specific files.

In the current code, gk20a.h includes io.h which gets directly included
in a lot of other files. io.h contains methods which uses a struct
gk20a as a parameter leading to a circular dependency between io.h
and gk20a.h. This can be mitigated by removing io.h from gk20a.h as
part of larger effort to moving gk20a.h to nvgpu/gk20a.h

Switch all logging to nvgpu_log*(). gk20a_dbg* macros are
intentionally left there because of use from other repositories.
Because the new functions do not work without a pointer to struct
gk20a, and piping it just for logging is excessive, some log messages
are deleted.

For gp10b and previous chips, RM server will issue fake mmu fault to
reset the channel. And the mmu fault event will be sent to vgpu client,
which will cause the client to abort the channel or tsg.
But on gv11b, RM server doesn't issue fake mmu fault any more. So I need
to abort it right after the force reset is finished.

Split the ctxsw trace "core" API code into <nvgpu/ctxsw_trace.h>. This
is not perect though since there's some Linuxisms present in the HAL
and as such that code has to be hidden by the ctxsw tracing CONFIG. But
this patch should work for QNX such that it will allow the code to
build as long as CONFIG_GK20A_CTXSW_TRACE is not set.
Also fix the copywrite notice in the ctxsw code present under
common/linux to be GPL.

In current code vgpu only set error notifier for the reporting channel
but abort the whole tsg. When the tsg is aborted, all channels of the
tsg are supposed have had their error notifiers set. Set it for all
channels once any of the channels gets an MMU fault.
For now, RM server still reports num-of-channel times for tsg mmu fault.
We may optimize it in future.

BAR1 test could occasionally fail when doing CPU write through userd
then reading back through BAR1. This is because nvgpu_smp_mb() only
guarantees ordering between cores.
Replaced with nvgpu_mb() to ensure the write will be visible to all
bus masters in the system.

Instead of calling the native HAL init function then adding
multiple layers of modification for VGPU, flatten out the sequence

Add abstraction of IO aperture accessors. Add new functions
gk20a_io_exists() and gk20a_io_valid_reg() to remove dependencies to
aperture fields from common code.
Implement Linux version of the abstraction by moving gk20a_readl()
and gk20a_writel() to new Linux specific io.c. Move the fields
defining IO aperture to nvgpu_os_linux.
Add t19x specific IO aperture initialization functions and add t19x
specific section to nvgpu_os_linux.

Remove the mm.get_iova_addr() HAL and replace it with a new HAL
called mm.gpu_phys_addr(). This new HAL provides the real phys
address that should be passed to the GPU from a physical address
obtained from a scatter list. It also provides a mechanism by
which the HAL code can add extra bits to a GPU physical address
based on the attributes passed in. This is necessary during GMMU
page table programming.
Also remove the flags argument from the various address functions.
This flag was used for adding an IO coherence bit to the GPU
physical address which is not supported.

nvgpu_log/info/warn/err() internally add a \n to the end of the message.
Hence, callers should not include a \n at the end of the message. Doing
so results in duplicate \n being printed, which ends up creating empty
log messages. Remove the duplicate \n from all err/warn messages.
