The high level API for the timer unit is the same across all OSs, so
get rid of the slight code duplication by moving the timer init
functions under a new file in common code:
- nvgpu_timeout_init_cpu_timer
- nvgpu_timeout_init_cpu_timer_sw
- nvgpu_timeout_init_retry
Much of the timer logic is also duplicated, but it is mixed between OS
specific current time retrieval. With some refactoring and addition of
an OS independent time keeping layer, that logic could also be made
shared.

The nvgpu timeout API has an internal override for presilicon mode by
default: in presi simulation environments the timeouts never trigger.
This behaviour is intended in the original usecase of the timer unit
with hardware polling loops. In pure software logic though, the timer
must trigger after the specified timeout even in presi mode so add a new
init function to produce a timer for software logic. Use this new kind
of timer in channel and scheduling worker threads.
The channel worker currently times out for just the purpose of the
channel watchdog timer which has its own internal timer. Although that's
just software, the general expectation is that the watchdog does not
trigger in presilicon tests that run slower than usual. The internal
watchdog timer thus keeps the non-sw mode.

nvgpu_timeout_init() returns an error code only when the flags parameter
is invalid. There are very few possible values for flags, so extract the
two most common cases - cpu clock based and a retry based timeout - to
functions that cannot fail and thus return nothing. Adjust all callers
to use those, simplfying error handling quite a bit.

MISRA Advisory Rule 2.5 states that a project should not contain unused
macro declarations.
While most of the violations in the nvgpu driver are due to unused
macros from hw headers, devinit-related headers, etc. there is a small
number that are due to things like:
* macros not being used when they could/should be
* macros in C files that are really not referenced
* CPP build flag mismatches
This change eliminates such violations from the following:
* replace constants with existing macros in timeout conversion code
* wrap nvgpu_gmmu_dbg macro #defines in #ifdef CONFIG_NVGPU_TRACE/#endif
* wrap MAX_MC_INTR_REGS #define in #ifdef CONFIG_NVGPU_NON_FUSA/#endif
* remove unused FECS_MAILBOX_0_ACK_RESTORE from runlist code
* wrap BACKTRACE_MAXSIZE macro with #ifndef _QNX_SOURCE/#endif

In the current logic for nvgpu_timeout_expired(), function always
returns 0 if fault injection is enabled. This only helps for testing
timeout not expired scenarios. However, if nvgpu_timeout_expired() is
used in a while(true) loop, it is impossible to break the infinite loop.
This patch modifies nvgpu_timeout_expired() to not expire until fault
injection counter is non-zero. The function will now return -ETIMEDOUT
when fault injection is enabled and counter is zero.
