MISRA rule 11.2 doesn't allow conversions of a pointer from or to an
incomplete type. These type of conversions may result in a pointer
aligned incorrectly and may further result in undefined behavior.
This patch addresses rule 11.2 violations related to pointers to and
from struct nvgpu_sgl. This patch replaces struct nvgpu_sgl pointers by
void pointers.

Currently, nvgpu_vidmem_buf_access_memory() accepts u64 size/offset
values to access memory. However, underlying nvgpu_mem read and write
functions truncate size/offset value to u32. So, any VIDMEM buffer
larger than 4GB will be inaccessible above 4GB by userspace IOCTL.
This patch updates nvgpu_mem_rd_n() and nvgpu_mem_wr_n() to accept
u64 size and u64 offset values.

Safety build does not support vidmem. This patch compiles out vidmem
related changes - vidmem, dma alloc, cbc/acr/pmu alloc based on
vidmem and corresponding tests like pramin, page allocator &
gmmu_map_unmap_vidmem..
As vidmem is applicable only in case of DGPUs the code is compiled
out using CONFIG_NVGPU_DGPU.

Make a hal/mm/gmmu sub-unit for the GMMU HAL code. Also move the
gk20a specific HAL code there. gp10b will happen in the next patch.
This change also updates all the GMMU related HAL usage, of which
there is quite a bit. Generally the only change is a .gmmu needs to
be inserted into the HAL path. Each HAL init was also updated.

Currently, in nvgpu_aperture_mask_raw function, if NVGPU_MM_HONORS_APERTURE
flag is disabled, invalid or junk aperture input is changed to APERTURE_VIDMEM
instead of raising a warning.To resolve this bug, need to check if input
aperture is APERTURE_INVALID or undefined.
This patch adds APERTURE_MAX_ENUM to nvgpu_aperture structure which
gives upperbound of nvgpu_aperture types. This patch also adds condition in
nvgpu_aperture_mask_raw function to check for invalid or undefined aperture
input. For invalid inputs, function will BUG().

Make a physical nvgpu_mem implementation in the common code. This
implementation assumes a single, contiguous, physical range. GMMU
mappability is provided by building a one entry SGT.
Since this is now "common" code the original Linux code has been
moved to commom/mm/nvgpu_mem.c.
Also fix the '__' prefix in the nvgpu_mem function. This is not
necessary as this function, although somewhat tricky, is expected
to be used by arbitrary users within the nvgpu driver.

Split the nvgpu_sgt code out from the nvgpu_mem code. Although the
two chunks of code are related the SGT code is distinct and as
such should be its own unit. To do this a new source file has been
added - nvgpu_sgt.c - which contains all the nvgpu_sgt common APIs.
These are the facade APIs to abstract the actual details of how any
given nvgpu_sgt is actually implemented.
An abstract unit - nvgpu_sgt_os - was also defined. This unit
exists solely for the nvgpu_sgt unit to call so that the OS
specific nvgpu_sgt_os_create_from_mem() API can be moved from the
common nvgpu_sgt unit. Note this also updates the name of what the
OS specific units are expected to call. Common code may still use
the generic nvgpu_sgt_create_from_mem() API.

MISRA Rule 21.15 prohibits use of memcpy() with incompatible ptrs
to qualified/unqualified types.
To circumvent this issue we've introduced a new MISRA-compliant
nvgpu_memcpy() function.
This change switches all offending uses of memcpy() in mm/*
over to use nvgpu_memcpy() with appropriate casts applied.

Sometimes longer 64-bit values are composed of pairs of low and high

Background:
In Hypervisor mode dGPU device is configured in pass through mode for
the Guest (QNX/Linux). GMMU programming is handled by the guest which
converts a mapped buffer's GVA into SGLes in IPA (Intermediate/Guest
Physical address) which is then translated into PA (Acutual Physical
address) and programs the GMMU PTEes with correct GVA to PA mapping.
Incase of the vgpu this work is delegated to the RM server which takes care
of the GMMU programming and IPA to PA conversion.
Problem:
The current GMMU mapping logic in the guest assumes that PA range is
continuous over a given IPA range. Hence, it doesn't account for holes being
present in the PA range. But this is not the case, a continous IPA range
can be mapped to dis-contiguous PA ranges. In this situation the mapping
logic sets up GMMU PTEes ignoring the holes in physical memory and
creates GVA => PA mapping which intrudes into the PA ranges which are
reserved. This results in memory being corrupted.
This change takes into account holes being present in a given PA range and
for a given IPA range it also identifies the discontiguous PA ranges and
sets up the PTE's appropriately.

MISRA 21.2 states that we may not use reserved identifiers; since
all identifiers beginning with '_' are reserved by libc, the usage
of '__' as a prefix is disallowed.
Handle the 21.2 fixes for nvgpu_mem.c and mm.c; this deletes the
'__' prefixes and slightly renames the __nvgpu_aperture_mask()
function since there's a coherent version and a general version.

MISRA Rule 10.4 only allows the usage of arithmetic operations on
operands of the same essential type category.
Adding "U" at the end of the integer literals to have same type of
operands when an arithmetic operation is performed.
This fix violations where an arithmetic operation is performed on
signed and unsigned int types.
In balloc_get_order_list() the argument "int order" has been changed to
a u64 because all callers of this function pass a u64 argument.

nvgpu_mem_rd*() functions were implemented per OS. They also used
nvgpu_pramin_access_batched() and implemented a big portion of logic
for using PRAMIN in OS specific code.
Make the implementation for the functions generic. Move all PRAMIN
logic to PRAMIN and simplify the interface provided by PRAMIN.

Allow a potential IOMMU'ed GMMU mapping for all SYSMEM buffers
inlcuding coherent sysmem. Typically this won't actually happen
since IO coherent mappings will also often be accessed over
NVLINK which is physically addressed.
Also update the comments surrounding this code to take into
account the new NVLINK nuances. Since NVLINK buffers are
directly mapped even when the IOMMU is enabled this is very
deserving of a comment explaining what's going on.
Lastly add some simple functions for checking if an nvgpu_mem
(or a particular aperture field) is a sysmem aperture. Currently
this includes SYSMEM and SYSMEM_COH.

The operations in struct nvgpu_sgt_ops have a scatter-gather list (sgl)
argument which is a void pointer. Change the type signatures to take
struct nvgpu_sgl * which is an opaque marker type that makes it more
difficult to pass around wrong arguments, as anything goes for void *.
Explicit types add also self-documentation to the code.
For some added safety, some explicit type casts are now required in
implementors of the nvgpu_sgt_ops interface when converting between the
general nvgpu_sgl type and implementation-specific types. This is not
purely a bad thing because the casts explain clearly where type
conversions are happening.

When using a coherent DMA API wee must make sure to program
any aperture fields with the coherent aperture setting. To
do this the nvgpu_aperture_mask() function was modified to
take a third aperture mask argument, a coherent setting, so
that code can use this function to generate coherent aperture
settings.
The aperture choice is some what tricky: the default version
of this function uses the state of the DMA API to determine
what aperture to use for SYSMEM: either coherent or
non-coherent internally. Thus a kernel user need only specify
the normal nvgpu_mem struct and the correct mask should be
chosen. Due to many uses of nvgpu_mem structs not created
directly from the DMA API wrapper it's easier to translate
SYSMEM to SYSMEM_COH after creation.
However, the GMMU mapping code, will encounter buffers from
userspace with difference coerency attributes than the DMA
API. Thus the __nvgpu_aperture_mask() really respects the
aperture setting passed in regardless of the DMA API state.
This aperture setting is pulled from NVGPU_VM_MAP_IO_COHERENT
since this is either passed in from userspace or set by the
kernel when using coherent DMA. The aperture field in attrs
is upgraded to coh if this flag is set.
This change also adds a coherent sysmem mask everywhere that
it can. There's a couple places that do not have a coherent
register field defined yet. These need to eventually be
defined and added.
Lastly the aperture mask code has been mvoed from the Linux
vm.c code to the general vm.c code since this function has
no Linux dependencies.

The GPU has multiple different operating modes in respect to IOMMU'ability.
As such there needs to be a clean way to tell the driver whether it is
IOMMU'able or not. This state also does not always reflect what is possible:
all becasue the GPU can generate IOMMU'ed memory requests doesn't mean it
wants to.
The nvgpu_iommuable() API has now existed for a little while which is a
useful way to convey whether nvgpu should consider the GPU as IOMMU'able.
However, there is also the g->mm.bypass_smmu flag which used to be able to
override what the GPU decided it should do. Typically it was assigned
the same value as nvgpu_iommuable() but that was not necessarily a
requirment.
This patch removes all the usages of g->mm.bypass_smmu and instead uses the
nvgpu_iommuable() function. All places where the check against
g->mm.bypass_smmu have been replaced with nvgpu_iommuable(). The code
should now be much cleaner.
Subsequently other checks can also be placed in the nvgpu_iommuable()
function. For example, when NVLINK comes online and the GPU should no
longer consider DMA addresses and instead use scatter-gather lists
directly the ngpu_iommuable() function will be able to check the state of
NVLINK and then act accordingly.

Drastically simplify and move the aligment computation for buffers
getting mapped into the SGT code. An SGT is all that is needed for
computing the alignment.
However, this did require that a new SGT op was added:
nvgpu_sgt_iommuable()
This function returns true if the passed SGT is IOMMU'able and must
be implemented by an SGT implementation that has IOMMU'able buffers.
If this function is left as NULL then it is assumed that the buffer
is not IOMMU'able.
Also cleanup the parameter ordering convention among all nvgpu_sgt
functions. Previously there was a mishmash of different parameter
orderings. This patch now standardizes on the gk20a first approach
seen everywhere else in the driver.

Split VIDMEM support into its own code files organized as such:
common/mm/vidmem.c - Base vidmem support
common/linux/vidmem.c - Linux specific user-space interaction
include/nvgpu/vidmem.h - Vidmem API definitions
Also use the config to enable/disable VIDMEM support in the makefile
and remove as many CONFIG_GK20A_VIDMEM preprocessor checks as possible
from the source code.
And lastly update a while-loop that iterated over an SGT to use the
new for_each construct for iterating over SGTs.
Currently this organization is not perfectly adhered to. More patches
will fix that.

Rename get_physical_addr_bits and related functions to something that
more clearly conveys what they are doing. The basic idea of these
functions is to translate from a physical GPU address to a IOMMU GPU
address. To do that a particular bit (that varies from chip to chip)
is added to the physical address.

The basic nvgpu_mem_sgl implementation provides support
for OS specific scatter-gather list implementations by
simply copying them node by node. This is inefficient,
taking extra time and memory.
This patch implements an nvgpu_mem_sgt struct to act as
a header which is inserted at the front of any scatter-
gather list implementation. This labels every struct
with a set of ops which can be used to interact with
the attached scatter gather list.
Since nvgpu common code only has to interact with these
function pointers, any sgl implementation can be used.
Initialization only requires the allocation of a single
struct, removing the need to copy or iterate through the
sgl being converted.
