Remove the second parameter for the pmu_early_init() and pmu_init()
functions so they only require the gk20a object. The g->pmu was always
passed for this parameter. And this makes the API signature match the
other init functions in the driver.

This patch addresses misra violations due to SDL error reporting
callbacks. In particular, it addresses the following misra violation:
- misra_c_2012_directive_4_7_violation: Calling function
"nvgpu_report_*_err()" which returns error information without testing
the error information.

Make the nvgpu_init_mutex function return void.
In linux case, this doesn't affect anything since mutex_init
returns void.
For posix, we assert() and die if pthread_mutex_init fails.
This alleviates the need to error inject for _every_
nvgpu_mutex_init function in the driver.

This patch moves gops init related to SDL from qnx to common-core. For this
purpose, it does the following changes:
- Adds stub functions for linux and posix.
- Updates nvgpu_init.c for mapping err_ops with report error APIs.
- Updates nvgpu_err.h header file to include prototypes related to error
reporting APIs.
- Updates nvgpu-linux.yaml file to include sdl_stub file.

Modified PMU subunits init sequence as they required during PMU RTOS
initialize stage, function nvgpu_pmu_early_init() allocates space for
all its subunit & its default values for further usage during runtime,
and function nvgpu_pmu_init() performs setup for sub units & starts
PMU-RTOS boot.
Deinit of all sub units allocated space will handled as part of
remove_support stage & also, during init stage upon failure of
init/setup sequence.

-Currently PG task is created for both iGPU & dGPU as part PMU init
sequence path, but task is not required for dGPU or can be skipped
if ELPG is not supported on iGPU, made changes to create PG task only
if supported else skip it, and made some functions to private as these
are required by PG UNIT only.
-PG instance is allocated & set to default properties as needed if
support is enabled else skip it.
-Made changes in dependent files as required to reflect above changes

Moved some struct from pmu header file to respective unit.
removed pmu_fw.c file, which is not required anymore, missed removing

Created PMU fw unit to hold PMU RTOS f/w specific ops, images,
flags & command arguments needed for PMU RTOS ucode support.
Moved PMU fw ops from gk20a.gpu_ops to pmu.fw.ops as these ops
are needed to support different version of PMU fw version for
different chips

- PG seq_buf is used to send command to PMU which is
related to ZBC and it is handled as part of PG state
machine path so moving seq_buf from struct nvgpu_pmu
to pmu_pg struct.
- Also, adding two new functions in PG unit:
nvgpu_pmu_pg_init_seq_buf() and nvgpu_pmu_pg_free_seq_buf()
to allocate and free up space for seq_buf respectively.

- Move the perfmon unit source code to common/pmu/perfmon/ folder
- Separate perfmon unit headers under include/nvgpu/pmu/pmu_perfmon.h
- Make a new structure: nvgpu_pmu_perfmon for perfmon unit
- This new struct combines all perfmon unit variables like
perfmon_query, perfmon_ready etc. into one
structure as a part of perfmon unit refactoring.
- Use pmu_perfmon struct to access all perfmon variables.
- Eg: pmu->pmu_perfmon->perfmon_query, pmu->pmu_perfmon->perfmon_ready

PMU message handling unit can't be part of command handling unit as it
creates circular dependencies with the PMU tasks (clk, therm etc.)
PMU allocator unit shall encompass DMEM allocator and other allocators
used by PMU.

PMU mutexes used by FIFO and runlists is functionality independent of
the PMU command and message management.
Remove related functionality from pmu_ipc.c and prepare pmu_mutex.c.
Prepare PMU HAL unit that contains gk20a specific PMU mutexes
handling.

PMU commands and messages management is based on sharing the data
through sequences. Functions for sending commands/allocating
payload update sequence data acquiring lock and those for
working on received messages read/free the sequence data
releasing lock.

Add new power/clock gating functions that can be called by
other units.
New clock_gating functions will reside in cg.c under
common/power_features/cg unit.
New power gating functions will reside in pg.c under
common/power_features/pg unit.
Use nvgpu_pg_elpg_disable and nvgpu_pg_elpg_enable to disable/enable
elpg and also in gr_gk20a_elpg_protected macro to access gr registers.
Add cg_pg_lock to make elpg_enabled, elcg_enabled, blcg_enabled
and slcg_enabled thread safe.

Added support to disable/skip to load LS PMU based on PMU support flag,
when LS PMU skipped only basic PMU engine ops are needed for HS ACR
to load & execute on PMU engine falcon,
GR LS falcons cold/recovery bootstrap will be taken care by ACR as HS
ACR will be loaded for both case & exits by halting in non-secure mode.

Currently PMU support enable check is done with multiple
methods which added complexity to know status of PMU
support.
Changed to replace multiple methods with support_pmu
flag to know the PMU support, support_pmu will be updated
at init stage based on platform/chip specific settings
to know the PMU support status.
Cleaned up support_pmu flag check with platform specific
PMU members in multiple places & moved check to
public functions

The type for the timeout parameter to the NVGPU_COND_WAIT and
NVGPU_COND_WAIT_INTERRUPTIBLE macros was too weak. This updates these
macros to require a u32 for the timeout.
Users of the macros are updated to be compliant as necessary.
This addresses MISRA 10.3 violations for implicit conversions of types
of different size or essential type.

As part of creating zbc as gr subunit, move pmu_save hal function
from zbc to pmu hal.
This hal function is used to pass the information to gpmu
firmware, which should reside as part of pmu.
remove pmu_save hal from zbc.
add save_zbc hal under pmu.
remove unused function gr_gk20a_pmu_save_zbc
